Each of the 13 senior living communities was founded on the principles of empathy, respect, and holistic well-being for older adults. Over the years, as these communities thrived and grew, a shared belief emerged – that everyone deserves compassionate care during their final days, regardless of where they reside.

To accomplish this goal, we have partnered with Midland Care Connection, a not-for-profit hospice provider with 45 years of experience providing hospice care. Midland will provide its expertise within hospice to develop and manage a hospice that provides individualized care within each facility to offer a comprehensive range of services tailored to the unique needs of individuals and their families facing life-limiting illnesses.

At Community Care Hospice, we believe in the power of multidisciplinary care, bringing together professionals from various fields to provide a comprehensive and holistic approach to end-of-life care.
